The Industrial Pretreatment Program requires that nonresidential sewer users of the Authority's sewerage system complete and return to the Authority the Authority's Nonresidential Sewer Use Application (TBSA Form C) and the Supplemental Non-Domestic Sewer Use Information (TBSA Form D), both which can be obtained by clicking here. Please completely fill out both forms and mail to:
